Faculty Request for Electronic / Copy Reserves This form is used by faculty to place electronic / copy reserves for students in support of classroom instruction.

Faculty Request for Book and Audio-Visual Reserves This form is used by faculty to place a reserve for books and audio visual material, for students in support of classroom instruction.

Library Holds If materials are on loan to another PCA&D patron and not available for check-out, a hold can be placed on the item to ensure that it will be held for you when it is returned to the library. A hold form must be completed and submitted to the librarian. The librarian will contact you when the book becomes available.

Interlibrary Loan After searching the Access PA database and locating a book not available in our college library collection, complete the Interlibrary request form and submit it to the librarian. The librarian will process your request and contact you when the book has arrived.

Faculty Request for Making Slides Faculty may request to have slides made to support classroom instruction. Please see the slide making policy before completing and submiting the form.

Request for Reconsideration of Library Resources Refer to the Intellictual Freedom portion of the Collection Development Policy before completing and submiting this form.

Faculty Request for Information Literacy Instruction In support of information literacy, faculty may request library instruction for their class.

Request to Purchase Material Faculty, students and staff requesting material to be added to the collection must complete and submit a Request to Purchase Material form. Please refer to the Collection Development Policy for a detailed explanation of the responsibilities and guidelines for the acquisition of material.
